I do: add a ratchet! For people who live in very gusty places, all the time up and down, and with less than ideal shores, going back to the beach is a pain (especially with a foil) so the Duotone power XT has been extremely handy. I envy those people who set the sail and don't touch outhaul/ downhaul unless they are changing sails!
I know you can change the outhaul setting in the water even with a regular extension, but when you start using the ratchet, you don't go back to a normal extension. Disclaimer: I use 8.5 m cambered sails down to 3.7 wave sails (for foiling). If I only used small sails I can see less of a benefit, but with big sails it is a no brainer.
