Which is a very valid concern. So, I have added a little on my safety line rig. Not flash, but I feel safe enough with it.
The tether is just long enough for me to stand at the mast.
I have to kneel on the foredeck to set up the bowsprit or gennaker set up.
I can go below with it connected.

Do you know why I always wear a harness and tether? I can't say for sure if it will save my life. But if I trip over a line and fall overboard, I don't want to be floating in the cold North Pacific, and slowly dying over the next 4 hours, while watching my boat sail away and thinking what an incredible idiot I was for not wearing it. That is not the last thought I want to have in this life.

I sail multis mostly offshore but I wonder if we have our safety harnesses set up all wrong. The idea is to get us to not fall overboard with a short tether, or to keep us attached to the boat with long one.

I do a fair bit of singlehanding and I use a tether when on my own. Cats are pretty good at setting out jacklines so it is harder to fall overboard and access the whole deck but I could fall overboard at some points. And then I am dragging next to the boat at up to 10 knots (or more). It will not be fun.

When I have sailed skiffs and dinghies I have had a lot of giggles when we stuff up on trapeze and end up dragging along next to the boat. No worries as we try to get the boat to head up so that it will bring us back towards the windward gunwale again. So why not use a similar idea for a harness?

You wear a climbing harness attached to a trapeze wire, set below the genoa halyard. There is a 3 to 1 and bungee on the end so that there is less slack and you can pull yourself up with the tackle (just like trapeze crew use).

I have to see how it would work for a cat but on a mono it would need some sort of way to get back aboard if you fell to leeward. At least the crew could get the boat flat and you would come swinging back in. It is pretty fun and would be much safer than dragging in the water next to the boat.

The answer to both of those questions is , yes.

In the video he has connected to the outer perimeter of the wheel and looks like a gerry rig first try. (Been there , done that)
This would give limited movement, but it seems to be working.
I would make up a 300mm to 450 mm ply disc and fit to the hub. Have lots of holes so you could experiment with cam cleat placement.
Don't see any reason that you could not get it working well.

Interesting thought, a sailing abseiling arrangement, a Tarzan swing tether, you could go for a stroll on the topsides.
