Just watched it again, still struggle with this movie. Can anybody shed some light on what this film is all about. I enjoy it but dont understand it.

It's probably better you see the Spanish version. I think that one made more sense, especially towards the end.
Basically, a wealthy young batchelor falls in love with a woman that his best friend also loves, but won't admit to it. His best friend is basically trying to steal his new flame. However, another girl he knows is jealous, so this rich guy has in effect two enemies or protaganists.
One of them is more sadistic, the ex girlfriend and tries to kill them both in a car crash. She dies, but he survives and is facially disfigured. Now, his best friend sees awesome opp here to slowly steal his girlfriend from him and does this by making his friend go paranoid, believing that he also suspects his best friend to be wanting to take his new flame away from him. But what his best friend does not realise is that she is in love with the batchelor too, even though he's disfigured.
Now comes the twist.
At some point, and it was the night the disfigured guy in paranoia 'feels' his new girlfirend has gone with his best friend and we see them kissing. He stumbles drunk, falls on the street and wakes up in a 'new' life, where there is hope. This new life is the virtual reality world, the Vanilla Sky where behind the blue sky is a computer screen or blank screen... Ie white.
The rest of the movie is basically bull**** to make him think he has recovered, a miracle was done on his face and it's fully restored and he wins his girlfriend back. However, towards the end he realises that he may not be completely restored as he has flashbacks and wakes up in nightmares thinking his gone back to his disfigured self.
In the meantime, his new girlfriend whom is still in love with him, goes into 'his dreams' in the vanilla world to go along with the lie, but only to try and hopefully restore his faith in her so he can continue living 'in altered state' if you will. This is so until a future time when medicine and science figure out a way to do miracle plastic surgey on his face and bring him back to reality.
We don't know what's really happening in the real world because we're not shown it. For all we know, the woman and his best friend could have actually married, had kids and grown old all while their friend is in the vanilla world dreaming things are going to get better.
In the end he committs 'suicide' to wake up from the system, and his real girlfriend is whispering into his ear, all black screen, '... Open your eyes', 'Open your eyes'.
It's a great movie I think.
